unix,win: merge timers implementation

Merge src/unix/timer.c and src/win/timer.c into src/timer.c. This
changes the Windows implementation from a binary tree to a binary
heap for generally better performance.

unix,win: limit uv_timer_again to repeating timers

This commit causes uv_timer_again() to return UV_EINVAL
for non-repeating timers. It also adds clarification to
the related documentation.
