cmake: add support for "unity" builds
Aka "jumbo" or "amalgamation" builds. It means to compile all sources
per target as a single C source. This is experimental.

You can enable it by passing `-DCMAKE_UNITY_BUILD=ON` to cmake.
It requires CMake 3.16 or newer.

It makes builds (much) faster, allows for better optimizations and tends
to promote less ambiguous code.

Also add a new AppVeyor CI job and convert an existing one to use
"unity" mode (one MSVC, one MinGW), and enable it for one macOS CI job.

Fix related issues:
- add missing include guard to `easy_lock.h`.
- rename static variables and functions (and a macro) with names reused
  across sources, or shadowed by local variables.
- add an `#undef` after use.
- add a missing `#undef` before use.
- move internal definitions from `ftp.h` to `ftp.c`.
- `curl_memory.h` fixes to make it work when included repeatedly.
- stop building/linking curlx bits twice for a static-mode curl tool.
  These caused doubly defined symbols in unity builds.
- silence missing extern declarations compiler warning for ` _CRT_glob`.
- fix extern declarations for `tool_freq` and `tool_isVistaOrGreater`.
- fix colliding static symbols in debug mode: `debugtime()` and
  `statename`.
- rename `ssl_backend_data` structure to unique names for each
  TLS-backend, along with the `ssl_connect_data` struct member
  referencing them. This required adding casts for each access.
- add workaround for missing `[P]UNICODE_STRING` types in certain Windows
  builds when compiling `lib/ldap.c`. To support "unity" builds, we had
  to enable `SCHANNEL_USE_BLACKLISTS` for Schannel (a Windows
  `schannel.h` option) _globally_. This caused an indirect inclusion of
  Windows `schannel.h` from `ldap.c` via `winldap.h` to have it enabled
  as well. This requires `[P]UNICODE_STRING` types, which is apperantly
  not defined automatically (as seen with both MSVS and mingw-w64).
  This patch includes `<subauth.h>` to fix it.

checksrc: disallow spaces before labels
Out of 415 labels throughout the code base, 86 of those labels were
not at the start of the line. Which means labels always at the start of
the line is the favoured style overall with 329 instances.

Out of the 86 labels not at the start of the line:
* 75 were indented with the same indentation level of the following line
* 8 were indented with exactly one space
* 2 were indented with one fewer indentation level then the following
  line
* 1 was indented with the indentation level of the following line minus
  three space (probably unintentional)

easy_cleanup: require a "good" handle to act
By insisting that the passed in handle is "good" (the magic number is
intact), this can limit the potential damage if a bad pointer is passed
in. Like when this function is called twice on the same handle pointer.

lib: connect/h2/h3 refactor
Refactoring of connection setup and happy eyeballing. Move
nghttp2. ngtcp2, quiche and msh3 into connection filters.

 - eyeballing cfilter that uses sub-filters for performing parallel connects
 - socket cfilter for all transport types, including QUIC
 - QUIC implementations in cfilter, can now participate in eyeballing
 - connection setup is more dynamic in order to adapt to what filter did
   really connect.  Relevant to see if a SSL filter needs to be added or
   if SSL has already been provided
 - HTTP/3 test cases similar to HTTP/2
 - multiuse of parallel transfers for HTTP/3, tested for ngtcp2 and quiche

 - Fix for data attach/detach in VTLS filters that could lead to crashes
   during parallel transfers.
 - Eliminating setup() methods in cfilters, no longer needed.
 - Improving Curl_conn_is_alive() to replace Curl_connalive() and
   integrated ssl alive checks into cfilter.
 - Adding CF_CNTRL_CONN_INFO_UPDATE to tell filters to update
   connection into and persist it at the easy handle.

 - Several more cfilter related cleanups and moves:
   - stream_weigth and dependency info is now wrapped in struct
     Curl_data_priority
   - Curl_data_priority members depend is available in HTTP2|HTTP3
   - Curl_data_priority members depend on NGHTTP2 support
   - handling init/reset/cleanup of priority part of url.c
   - data->state.priority same struct, but shallow copy for compares only

 - PROTOPT_STREAM has been removed
   - Curl_conn_is_mulitplex() now available to check on capability

 - Adding query method to connection filters.
   - ngtcp2+quiche: implementing query for max concurrent transfers.

 - Adding is_alive and keep_alive cfilter methods. Adding DATA_SETUP event.
   - setting keepalive timestamp on connect
   - DATA_SETUP is called after the connection has been completely
     setup (but may not connected yet) to allow filters to initialize
     data members they use.

 - there is no socket to be had with msh3, it is unclear how select
   shall work

 - manual test via "curl --http3 https://curl.se" fail with "empty
   reply from server".

 - Various socket/conn related cleanups:
   - Curl_socket is now Curl_socket_open and in cf-socket.c
   - Curl_closesocket is now Curl_socket_close and in cf-socket.c
   - Curl_ssl_use has been replaced with Cur_conn_is_ssl
   - Curl_conn_tcp_accepted_set has been split into
     Curl_conn_tcp_listen_set and Curl_conn_tcp_accepted_set
     with a clearer purpose

urldata: move the cookefilelist to the 'set' struct
The cookiefile entries are set into the handle and should remain set for
the lifetime of the handle so that duplicating it also duplicates the
list. Therefore, the struct field is moved from 'state' to 'set'.

mime: relax easy/mime structures binding
Deprecation and removal of codeset conversion support from the library
have released the strict need for an early binding of mime structures to
an easy handle (https://github.com/curl/curl/commit/2610142).

This constraint currently forces to create the handle before the mime
structure and the latter cannot be attached to another handle once
created (see https://curl.se/mail/lib-2022-08/0027.html).

This commit removes the handle pointers from the mime structures
allowing more flexibility on their use.

When an easy handle is duplicated, bound mime structures must however
still be duplicated too as their components hold send-time dynamic
information.

websockets: remodeled API to support 63 bit frame sizes
curl_ws_recv() now receives data to fill up the provided buffer, but can
return a partial fragment. The function now also get a pointer to a
curl_ws_frame struct with metadata that also mentions the offset and
total size of the fragment (of which you might be receiving a smaller
piece). This way, large incoming fragments will be "streamed" to the
application. When the curl_ws_frame struct field 'bytesleft' is 0, the
final fragment piece has been delivered.

curl_ws_recv() was also adjusted to work with a buffer size smaller than
the fragment size. (Possibly needless to say as the fragment size can
now be 63 bit large).

curl_ws_send() now supports sending a piece of a fragment, in a
streaming manner, in addition to sending the entire fragment in a single
call if it is small enough. To send a huge fragment, curl_ws_send() can
be used to send it in many small calls by first telling libcurl about
the total expected fragment size, and then send the payload in N number
of separate invokes and libcurl will stream those over the wire.

The struct curl_ws_meta() returns is now called 'curl_ws_frame' and it
has been extended with two new fields: *offset* and *bytesleft*. To help
describe the passed on data chunk when a fragment is delivered in many
smaller pieces.

The documentation has been updated accordingly.

Have curl_multi_init() use a much larger DNS hash table than used for
the easy interface to scale and perform better when used with _many_
host names.

curl_share_init() sets an in-between size.

easy: during upkeep, attach Curl_easy to connections in the cache
During the protocol-specific parts of connection upkeep, some code
assumes that the data->conn pointer already is set correctly.  However,
there's currently no guarantee of that in the code.

This fix temporarily attaches each connection to the Curl_easy object
before performing the protocol-specific connection check on it, in a
similar manner to the connection checking in extract_if_dead().

infof: remove newline from format strings, always append it
- the data needs to be "line-based" anyway since it's also passed to the
  debug callback/application

- it makes infof() work like failf() and consistency is good

- there's an assert that triggers on newlines in the format string

- Also removes a few instances of "..."

- Removes the code that would append "..." to the end of the data *iff*
  it was truncated in infof()

While working on documenting the states it dawned on me that step one is
to use more descriptive names on the states. This also changes prefix on
the states to make them shorter in the source.

State names NOT ending with *ing are transitional ones.

lib: pass in 'struct Curl_easy *' to most functions
... in most cases instead of 'struct connectdata *' but in some cases in
addition to.

- We mostly operate on transfers and not connections.

- We need the transfer handle to log, store data and more. Everything in
  libcurl is driven by a transfer (the CURL * in the public API).

- This work clarifies and separates the transfers from the connections
  better.

- We should avoid "conn->data". Since individual connections can be used
  by many transfers when multiplexing, making sure that conn->data
  points to the current and correct transfer at all times is difficult
  and has been notoriously error-prone over the years. The goal is to
  ultimately remove the conn->data pointer for this reason.

speedcheck: exclude paused transfers
Paused transfers should not be stopped due to slow speed even when
CURLOPT_LOW_SPEED_LIMIT is set. Additionally, the slow speed timer is
now reset when the transfer is unpaused - as otherwise it would easily
just trigger immediately after unpausing.

altsvc: clone setting in curl_easy_duphandle
The cache content is not duplicated, like other caches, but the setting
and specified file name are.

Test 1908 is extended to verify this somewhat. Since the duplicated
handle gets the same file name, the test unfortunately overwrites the
same file twice (with different contents) which makes it hard to check
automatically.

timeouts: move ms timeouts to timediff_t from int and long
Now that all functions in select.[ch] take timediff_t instead
of the limited int or long, we can remove type conversions
and related preprocessor checks to silence compiler warnings.

url: alloc the download buffer at transfer start
... and free it as soon as the transfer is done. It removes the extra
alloc when a new size is set with setopt() and reduces memory for unused
easy handles.

In addition: the closure_handle now doesn't use an allocated buffer at
all but the smallest supported size as a stack based one.

dynbuf: introduce internal generic dynamic buffer functions
A common set of functions instead of many separate implementations for
creating buffers that can grow when appending data to them. Existing
functionality has been ported over.

In my early basic testing, the total number of allocations seem at
roughly the same amount as before, possibly a few less.

3bfda07004 easy: Fix curl_easy_duphandle for builds missing IPv6 that use c-ares
- Ignore CURLE_NOT_BUILT_IN errors returned by c-ares functions in
  curl_easy_duphandle.

Prior to this change if c-ares was used as the resolver backend and
either it was too old or libcurl was built without IPv6 support then
some of our resolver functions could return CURLE_NOT_BUILT_IN to
curl_easy_duphandle causing it to fail.

mime: latch last read callback status.
In case a read callback returns a status (pause, abort, eof,
error) instead of a byte count, drain the bytes read so far but
remember this status for further processing.
Takes care of not losing data when pausing, and properly resume a
paused mime structure when requested.
New tests 670-673 check unpausing cases, with easy or multi
interface and mime or form api.

http2: make pausing/unpausing set/clear local stream window
This reduces the HTTP/2 window size to 32 MB since libcurl might have to
buffer up to this amount of data in memory and yet we don't want it set
lower to potentially impact tranfer performance on high speed networks.

Requires nghttp2 commit b3f85e2daa629
(https://github.com/nghttp2/nghttp2/pull/1444) to work properly, to end
up in the next release after 1.40.0.

global_init: move the IPv6 works status bool to multi handle
Previously it was stored in a global state which contributed to
curl_global_init's thread unsafety. This boolean is now instead figured
out in curl_multi_init() and stored in the multi handle. Less effective,
but thread safe.

global_init: assume the EINTR bit by default
- Removed from global_init since it isn't thread-safe. The symbol will
  still remain to not break compiles, it just won't have any effect going
  forward.

- make the internals NOT loop on EINTR (the opposite from previously).
  It only risks returning from the select/poll/wait functions early, and that
  should be risk-free.

curl_multi_poll: a sister to curl_multi_wait() that waits more
Repeatedly we see problems where using curl_multi_wait() is difficult or
just awkward because if it has no file descriptor to wait for
internally, it returns immediately and leaves it to the caller to wait
for a small amount of time in order to avoid occasional busy-looping.

This is often missed or misunderstood, leading to underperforming
applications.

This change introduces curl_multi_poll() as a replacement drop-in
function that accepts the exact same set of arguments. This function
works identically to curl_multi_wait() - EXCEPT - for the case when
there's nothing to wait for internally, as then this function will by
itself wait for a "suitable" short time before it returns. This
effectiely avoids all risks of busy-looping and should also make it less
likely that apps "over-wait".

This also changes the curl tool to use this funtion internally when
doing parallel transfers and changes curl_easy_perform() to use it
internally.

78ed3abe11 easy: resize receive buffer on easy handle reset
- In curl_easy_reset attempt to resize the receive buffer to its default
  size. If realloc fails then continue using the previous size.

Prior to this change curl_easy_reset did not properly handle resetting
the receive buffer (data->state.buffer). It reset the variable holding
its size (data->set.buffer_size) to the default size (READBUFFER_SIZE)
but then did not actually resize the buffer. If a user resized the
buffer by using CURLOPT_BUFFERSIZE to set the size smaller than the
default, later called curl_easy_reset and attempted to reuse the handle
then a heap overflow would very likely occur during that handle's next
transfer.

f26bc29cfe easy: fix win32 init to work without CURL_GLOBAL_WIN32
- Change the behavior of win32_init so that the required initialization
  procedures are not affected by CURL_GLOBAL_WIN32 flag.

libcurl via curl_global_init supports initializing for win32 with an
optional flag CURL_GLOBAL_WIN32, which if omitted was meant to stop
Winsock initialization. It did so internally by skipping win32_init()
when that flag was set. Since then win32_init() has been expanded to
include required initialization routines that are separate from
Winsock and therefore must be called in all cases. This commit fixes
it so that CURL_GLOBAL_WIN32 only controls the optional win32
initialization (which is Winsock initialization, according to our doc).

The only users affected by this change are those that don't pass
CURL_GLOBAL_WIN32 to curl_global_init. For them this commit removes the
risk of a potential crash.

urldata: rename easy_conn to just conn
We use "conn" everywhere to be a pointer to the connection.

Introduces two functions that "attaches" and "detaches" the connection
to and from the transfer.

Going forward, we should favour using "data->conn" (since a transfer
always only has a single connection or none at all) to "conn->data"
(since a connection can have none, one or many transfers associated with
it and updating conn->data to be correct is error prone and a frequent
reason for internal issues).

curl_easy_perform: fix timeout handling
curl_multi_wait() was erroneously used from within
curl_easy_perform(). It could lead to it believing there was no socket
to wait for and then instead sleep for a while instead of monitoring the
socket and then miss acting on that activity as swiftly as it should
(causing an up to 1000 ms delay).
