tidy-up: delete, comment or scope C macros reported unused
To reduce the number `-Wunused-macro` compiler warnings:
- delete unused macros.
- comment out unused macro that are part of a set.
- move macros into the scope they are used.

This may be useful to enable by default, but there are tricky cases that
I didn't manage to fix and paused the effort. E.g. internal features
checks in `openssl.c`. There is more, once those are fixed.

strtok: use namespaced strtok_r macro instead of redefining it
krb5 defines `strtok_r` for Windows unconditionally in its public
header:
dc5554394e/src/include/win-mac.h (L214-L215)
resulting in this warning:
```
lib\strtok.h(31,9): warning C4005: 'strtok_r': macro redefinition
      C:\vcpkg\installed\x64-windows\include\win-mac.h(215,9):
      see previous definition of 'strtok_r'
```

The krb5 macro collides with curl's internal definition, in case
the `strtok_r` function is undetected and falling back to a local
replacement.

tests: delete libhostname.so and chkhostname
Before this patch, `libhostname.so` and `chkhostname` were a test
facility for overriding `gethostname()` in non-debug builds on
Linux and other Unix platforms supporting `LD_PRELOAD`.

`gethostname()` has a single use with SMTP.

The alternative way to override `gethostname()` is building in debug
mode, which allows to do this via the `CURL_GETHOSTNAME` env, on all
platforms.

Drop the `LD_PRELOAD` solution in favour of the above.

Also:
- delete inactive NTLM code with a `gethostname()` call made from it.
- streamline NTLM code by dropping a `printf()` and a macro.
- tests: stop setting `CURL_GETHOSTNAME` where unnecessary.

spnego_gssapi: implement TLS channel bindings for openssl
Channel Bindings are used to tie the session context to a specific TLS
channel. This is to provide additional proof of valid identity,
mitigating authentication relay attacks.

Major web servers have the ability to require (None/Accept/Require)
GSSAPI channel binding, rendering Curl unable to connect to such
websites unless support for channel bindings is implemented.

IIS calls this feature Extended Protection (EPA), which is used in
Enterprise environments using Kerberos for authentication.

This change require krb5 >= 1.19, otherwise channel bindings won't be
forwarded through SPNEGO.

build: enable missing OpenSSF-recommended warnings, with fixes
https://best.openssf.org/Compiler-Hardening-Guides/Compiler-Options-Hardening-Guide-for-C-and-C++.html
as of 2023-11-29 [1].

Enable new recommended warnings (except `-Wsign-conversion`):

- enable `-Wformat=2` for clang (in both cmake and autotools).
- add `CURL_PRINTF()` internal attribute and mark functions accepting
  printf arguments with it. This is a copy of existing
  `CURL_TEMP_PRINTF()` but using `__printf__` to make it compatible
  with redefinting the `printf` symbol:
  https://gcc.gnu.org/onlinedocs/gcc-3.0.4/gcc_5.html#SEC94
- fix `CURL_PRINTF()` and existing `CURL_TEMP_PRINTF()` for
  mingw-w64 and enable it on this platform.
- enable `-Wimplicit-fallthrough`.
- enable `-Wtrampolines`.
- add `-Wsign-conversion` commented with a FIXME.
- cmake: enable `-pedantic-errors` the way we do it with autotools.
  Follow-up to d5c0351055 #2747
- lib/curl_trc.h: use `CURL_FORMAT()`, this also fixes it to enable format
  checks. Previously it was always disabled due to the internal `printf`
  macro.

Fix them:

- fix bug where an `set_ipv6_v6only()` call was missed in builds with
  `--disable-verbose` / `CURL_DISABLE_VERBOSE_STRINGS=ON`.
- add internal `FALLTHROUGH()` macro.
- replace obsolete fall-through comments with `FALLTHROUGH()`.
- fix fallthrough markups: Delete redundant ones (showing up as
  warnings in most cases). Add missing ones. Fix indentation.
- silence `-Wformat-nonliteral` warnings with llvm/clang.
- fix one `-Wformat-nonliteral` warning.
- fix new `-Wformat` and `-Wformat-security` warnings.
- fix `CURL_FORMAT_SOCKET_T` value for mingw-w64. Also move its
  definition to `lib/curl_setup.h` allowing use in `tests/server`.
- lib: fix two wrongly passed string arguments in log outputs.
  Co-authored-by: Jay Satiro

build: add more picky warnings and fix them
Enable more picky compiler warnings. I've found these options in the
nghttp3 project when implementing the CMake quick picky warning
functionality for it [1].

`-Wunused-macros` was too noisy to keep around, but fixed a few issues
it revealed while testing.

- autotools: reflect the more precisely-versioned clang warnings.
  Follow-up to 033f8e2a08 #12324
- autotools: sync between clang and gcc the way we set `no-multichar`.
- autotools: avoid setting `-Wstrict-aliasing=3` twice.
- autotools: disable `-Wmissing-noreturn` for MSYS gcc targets [2].
  It triggers in libtool-generated stub code.

- lib/timeval: delete a redundant `!MSDOS` guard from a `WIN32` branch.

- lib/curl_setup.h: delete duplicate declaration for `fileno`.
  Added in initial commit ae1912cb0d
  (1999-12-29). This suggests this may not be needed anymore, but if
  it does, we may restore this for those specific (non-Windows) systems.
- lib: delete unused macro `FTP_BUFFER_ALLOCSIZE` since
  c1d6fe2aaa.
- lib: delete unused macro `isxdigit_ascii` since
  f65f750742.
- lib/mqtt: delete unused macro `MQTT_HEADER_LEN`.
- lib/multi: delete unused macro `SH_READ`/`SH_WRITE`.
- lib/hostip: add `noreturn` function attribute via new `CURL_NORETURN`
  macro.
- lib/mprintf: delete duplicate declaration for `Curl_dyn_vprintf`.
- lib/rand: fix `-Wunreachable-code` and related fallouts [3].
- lib/setopt: fix `-Wunreachable-code-break`.
- lib/system_win32 and lib/timeval: fix double declarations for
  `Curl_freq` and `Curl_isVistaOrGreater` in CMake UNITY mode [4].
- lib/warnless: fix double declarations in CMake UNITY mode [5].
  This was due to force-disabling the header guard of `warnless.h` to
  to reapply it to source code coming after `warnless.c` in UNITY
  builds. This reapplied declarations too, causing the warnings.
  Solved by adding a header guard for the lines that actually need
  to be reapplied.
- lib/vauth/digest: fix `-Wunreachable-code-break` [6].
- lib/vssh/libssh2: fix `-Wunreachable-code-break` and delete redundant
  block.
- lib/vtls/sectransp: fix `-Wunreachable-code-break` [7].
- lib/vtls/sectransp: suppress `-Wunreachable-code`.
  Detected in `else` branches of dynamic feature checks, with results
  known at compile-time, e.g.
  ```c
  if(SecCertificateCopySubjectSummary)  /* -> true */
  ```
  Likely fixable as a separate micro-project, but given SecureTransport
  is deprecated anyway, let's just silence these locally.
- src/tool_help: delete duplicate declaration for `helptext`.
- src/tool_xattr: fix `-Wunreachable-code`.
- tests: delete duplicate declaration for `unitfail` [8].
- tests: delete duplicate declaration for `strncasecompare`.
- tests/libtest: delete duplicate declaration for `gethostname`.
  Originally added in 687df5c8c3
  (2010-08-02).
  Got complicated later: c49e9683b8
  If there are still systems around with warnings, we may restore the
  prototype, but limited for those systems.
- tests/lib2305: delete duplicate declaration for
  `libtest_debug_config`.
- tests/h2-download: fix `-Wunreachable-code-break`.

7703ca7f86 digest: Use hostname to generate spn instead of realm
In https://www.rfc-editor.org/rfc/rfc2831#section-2.1.2

digest-uri-value should be serv-type "/" host , where host is:

      The DNS host name or IP address for the service requested. The
      DNS host name must be the fully-qualified canonical name of the
      host. The DNS host name is the preferred form; see notes on server
      processing of the digest-uri.

Realm may not be the host, so we must specify the host explicitly.

Note this change only affects the non-SSPI digest code. The digest code
used by SSPI builds already uses the hostname to generate the spn.

openldap: fix missing sasl symbols at build in specific configs
If curl is built with openldap support (USE_OPENLDAP=1) but does not
have also some other protocol (IMAP/SMTP/POP3) enabled that brings
in Curl_sasl_* functions, then the build will fail with undefined
references to various symbols:

ld: ../lib/.libs/libcurl.so: undefined reference to `Curl_sasl_decode_mech'
ld: ../lib/.libs/libcurl.so: undefined reference to `Curl_sasl_parse_url_auth_option'
ld: ../lib/.libs/libcurl.so: undefined reference to `Curl_sasl_cleanup'
ld: ../lib/.libs/libcurl.so: undefined reference to `Curl_sasl_can_authenticate'
ld: ../lib/.libs/libcurl.so: undefined reference to `Curl_sasl_continue'
ld: ../lib/.libs/libcurl.so: undefined reference to `Curl_sasl_start'
ld: ../lib/.libs/libcurl.so: undefined reference to `Curl_sasl_init'

This was tracked down to these functions bein used in openldap.c but
defined in curl_sasl.c and then forward in two vauth/ files to have
a guard against a set of #define configurations that was now extended
to cover also this case.

Example configuration targeted that could reproduce the problem:

curl 7.87.1-DEV () libcurl/7.87.1-DEV .... OpenLDAP/2.6.3
Protocols: file ftp ftps http https ldap ldaps

misc: ISSPACE() => ISBLANK()
Instances of ISSPACE() use that should rather use ISBLANK(). I think
somewhat carelessly used because it sounds as if it checks for space or
whitespace, but also includes %0a to %0d.

For parsing purposes, we should only accept what we must and not be
overly liberal. It leads to surprises and surprises lead to bad things.
