lib: graceful connection shutdown
When libcurl discards a connection there are two phases this may go
through: "shutdown" and "closing". If a connection is aborted, the
shutdown phase is skipped and it is closed right away.

The connection filters attached to the connection implement the phases
in their `do_shutdown()` and `do_close()` callbacks. Filters carry now a
`shutdown` flags next to `connected` to keep track of the shutdown
operation.

Filters are shut down from top to bottom. If a filter is not connected,
its shutdown is skipped. Notable filters that *do* something during
shutdown are HTTP/2 and TLS. HTTP/2 sends the GOAWAY frame. TLS sends
its close notify and expects to receive a close notify from the server.

As sends and receives may EAGAIN on the network, a shutdown is often not
successful right away and needs to poll the connection's socket(s). To
facilitate this, such connections are placed on a new shutdown list
inside the connection cache.

Since managing this list requires the cooperation of a multi handle,
only the connection cache belonging to a multi handle is used. If a
connection was in another cache when being discarded, it is removed
there and added to the multi's cache. If no multi handle is available at
that time, the connection is shutdown and closed in a one-time,
best-effort attempt.

When a multi handle is destroyed, all connection still on the shutdown
list are discarded with a final shutdown attempt and close. In curl
debug builds, the environment variable `CURL_GRACEFUL_SHUTDOWN` can be
set to make this graceful with a timeout in milliseconds given by the
variable.

The shutdown list is limited to the max number of connections configured
for a multi cache. Set via CURLMOPT_MAX_TOTAL_CONNECTIONS. When the
limit is reached, the oldest connection on the shutdown list is
discarded.

- In multi_wait() and multi_waitfds(), collect all connection caches
  involved (each transfer might carry its own) into a temporary list.
  Let each connection cache on the list contribute sockets and
  POLLIN/OUT events it's connections are waiting for.

- in multi_perform() collect the connection caches the same way and let
  them peform their maintenance. This will make another non-blocking
  attempt to shutdown all connections on its shutdown list.

- for event based multis (multi->socket_cb set), add the sockets and
  their poll events via the callback. When `multi_socket()` is invoked
  for a socket not known by an active transfer, forward this to the
  multi's cache for processing. On closing a connection, remove its
  socket(s) via the callback.

TLS connection filters MUST NOT send close nofity messages in their
`do_close()` implementation. The reason is that a TLS close notify
signals a success. When a connection is aborted and skips its shutdown
phase, the server needs to see a missing close notify to detect
something has gone wrong.

A graceful shutdown of FTP's data connection is performed implicitly
before regarding the upload/download as complete and continuing on the
control connection. For FTP without TLS, there is just the socket close
happening. But with TLS, the sent/received close notify signals that the
transfer is complete and healthy. Servers like `vsftpd` verify that and
reject uploads without a TLS close notify.

- added test_19_* for shutdown related tests
- test_19_01 and test_19_02 test for TCP RST packets
  which happen without a graceful shutdown and should
  no longer appear otherwise.
- add test_19_03 for handling shutdowns by the server
- add test_19_04 for handling shutdowns by curl
- add test_19_05 for event based shutdowny by server
- add test_30_06/07 and test_31_06/07 for shutdown checks
  on FTP up- and downloads.

connection: shutdown TLS (for FTP) better
This adds connection shutdown infrastructure and first use for FTP. FTP
data connections, when not encountering an error, are now shut down in a
blocking way with a 2sec timeout.

    - add cfilter `Curl_cft_shutdown` callback
    - keep a shutdown start timestamp and timeout at connectdata
    - provide shutdown timeout default and member in
      `data->set.shutdowntimeout`.
    - provide methods for starting, interrogating and clearing
      shutdown timers
    - provide `Curl_conn_shutdown_blocking()` to shutdown the
      `sockindex` filter chain in a blocking way. Use that in FTP.
    - add `Curl_conn_cf_poll()` to wait for socket events during
      shutdown of a connection filter chain.
      This gets the monitoring sockets and events via the filters
      "adjust_pollset()" methods. This gives correct behaviour when
      shutting down a TLS connection through a HTTP/2 proxy.
    - Implement shutdown for all socket filters
      - for HTTP/2 and h2 proxying to send GOAWAY
      - for TLS backends to the best of their capabilities
      - for tcp socket filter to make a final, nonblocking
        receive to avoid unwanted RST states
    - add shutdown forwarding to happy eyeballers and
      https connect ballers when applicable.

openssl/gnutls: rectify the TLS version checks for QUIC
The versions check wrongly complained and return error if the *minimum*
version was set to something less than 1.3. QUIC is always TLS 1.3, but
that means minimum 1.2 is still fine to ask for.

This also renames the local variable to make the mistake harder to make
in the future.

02b14378e6 openssl: stop duplicate ssl key logging for legacy OpenSSL
- Don't call the keylog function if it has already logged the key.

For old OpenSSL versions and its forks that do not have support for
OpenSSL's keylog callback, libcurl has its own legacy key logging
function that logs the TLS 1.2 (and earlier) key (client random + master
key) on a single line.

Prior to this change, since e7de80e8 (precedes 8.8.0), the legacy key
logging function could write the same key line more than once (usually
twice) due to some incorrect logic.

9aae9bf817 openssl: revert keylog_callback support for LibreSSL
- Revert to the legacy TLS 1.2 key logging code for LibreSSL.

- Document SSLKEYLOGFILE for LibreSSL is TLS 1.2 max.

Prior to this change if the user specified a filename in the
SSLKEYLOGFILE environment variable and was using LibreSSL 3.5.0+ then
an empty file would be created and no keys would be logged.

This is effectively a revert of e43474b4 which changed openssl.c to use
SSL_CTX_set_keylog_callback for LibreSSL 3.5.0+. Unfortunately LibreSSL
added that function only as a stub that doesn't actually do anything.

build: prefer USE_IPV6 macro internally (was: ENABLE_IPV6)
Before this patch, two macros were used to guard IPv6 features in curl
sources: `ENABLE_IPV6` and `USE_IPV6`. This patch makes the source use
the latter for consistency with other similar switches.

`-DENABLE_IPV6` remains accepted for compatibility as a synonym for
`-DUSE_IPV6`, when passed to the compiler.

`ENABLE_IPV6` also remains the name of the CMake and `Makefile.vc`
options to control this feature.

- Use data->multi and not data->multi_easy to refer to the active multi.

The easy handle's active multi is always data->multi.

This is a follow up to 757dfdf which changed curl so that an easy handle
used with the easy interface and then multi interface cannot have two
different multi handles associated with it at the same time
(data->multi_easy from the easy interface and data->multi from the multi
interface).

TLS: start shutdown only when peer did not already close
- When curl sees a TCP close from the peer, do not start a TLS shutdown.
  TLS shutdown is a handshake and if the peer already closed the
  connection, it is not interested in participating.

Enable new recommended warnings (except `-Wsign-conversion`):

- enable `-Wformat=2` for clang (in both cmake and autotools).
- add `CURL_PRINTF()` internal attribute and mark functions accepting
  printf arguments with it. This is a copy of existing
  `CURL_TEMP_PRINTF()` but using `__printf__` to make it compatible
  with redefinting the `printf` symbol:
  https://gcc.gnu.org/onlinedocs/gcc-3.0.4/gcc_5.html#SEC94
- fix `CURL_PRINTF()` and existing `CURL_TEMP_PRINTF()` for
  mingw-w64 and enable it on this platform.

  checks. Previously it was always disabled due to the internal `printf`
  macro.

Fix them:

- fix bug where an `set_ipv6_v6only()` call was missed in builds with
  `--disable-verbose` / `CURL_DISABLE_VERBOSE_STRINGS=ON`.
- add internal `FALLTHROUGH()` macro.
- replace obsolete fall-through comments with `FALLTHROUGH()`.
- fix fallthrough markups: Delete redundant ones (showing up as

openssl: enable infof_certstack for 1.1 and LibreSSL 3.6
Lower the barrier to enable `infof_certstack()` from OpenSSL 3 to
OpenSSL 1.1.x, and LibreSSL 3.6 or upper.

With the caveat, that "group name" and "type name" are missing from
the log output with these TLS backends.

openssl: fix building with v3 no-deprecated + add CI test
- build quictls with `no-deprecated` in CI to have test coverage for
  this OpenSSL 3 configuration.

- don't call `OpenSSL_add_all_algorithms()`, `OpenSSL_add_all_digests()`.
  The caller code is meant for OpenSSL 3, while these two functions were
  only necessary before OpenSSL 1.1.0. They are missing from OpenSSL 3
  if built with option `no-deprecated`, causing build errors:

tidy-up: use OPENSSL_VERSION_NUMBER
Uniformly use `OPENSSL_VERSION_NUMBER` to check for OpenSSL version.
Before this patch some places used `OPENSSL_VERSION_MAJOR`.

Also fix `lib/md4.c`, which included `opensslconf.h`, but that doesn't
define any version number in these implementations: BoringSSL, AWS-LC,
LibreSSL, wolfSSL. (Only in mainline OpenSSL/quictls). Switch that to
`opensslv.h`. This wasn't causing a deeper problem because the code is
looking for v3, which is only provided by OpenSSL/quictls as of now.

According to https://github.com/openssl/openssl/issues/17517, the macro
`OPENSSL_VERSION_NUMBER` is safe to use and not deprecated.

openssl: identify the "quictls" backend correctly
Since vanilla OpenSSL does not support the QUIC API I think it helps
users to identify the correct OpenSSL fork in version output. The best
(crude) way to do that right now seems to be to check if ngtcp2 support
is enabled.

2d6333101a openssl: make CURLSSLOPT_NATIVE_CA import Windows intermediate CAs
- If CURLSSLOPT_NATIVE_CA on Windows then import from intermediate CA
  "CA" store after importing from root CA "ROOT" store.

This change allows curl to work in situations where a server does not
send all intermediate certs and they are present in the "CA" store (the
store with intermediate CAs). This is already allowed by the Schannel
backend.

Also this change makes partial chain verification possible for those
certs since we allow partial chain verification by default for OpenSSL
(unless CURLSSLOPT_NO_PARTIALCHAIN). This is not allowed by the Schannel
backend.

Prior to this change CURLSSLOPT_NATIVE_CA only imported "ROOT" certs.

lib: introduce struct easy_poll_set for poll information
Connection filter had a `get_select_socks()` method, inspired by the
various `getsocks` functions involved during the lifetime of a
transfer. These, depending on transfer state (CONNECT/DO/DONE/ etc.),
return sockets to monitor and flag if this shall be done for POLLIN
and/or POLLOUT.

Due to this design, sockets and flags could only be added, not
removed. This led to problems in filters like HTTP/2 where flow control
prohibits the sending of data until the peer increases the flow
window. The general transfer loop wants to write, adds POLLOUT, the
socket is writeable but no data can be written.

This leads to cpu busy loops. To prevent that, HTTP/2 did set the
`SEND_HOLD` flag of such a blocked transfer, so the transfer loop cedes
further attempts. This works if only one such filter is involved. If a
HTTP/2 transfer goes through a HTTP/2 proxy, two filters are
setting/clearing this flag and may step on each other's toes.

Connection filters `get_select_socks()` is replaced by
`adjust_pollset()`. They get passed a `struct easy_pollset` that keeps
up to `MAX_SOCKSPEREASYHANDLE` sockets and their `POLLIN|POLLOUT`
flags. This struct is initialized in `multi_getsock()` by calling the
various `getsocks()` implementations based on transfer state, as before.

After protocol handlers/transfer loop have set the sockets and flags
they want, the `easy_pollset` is *always* passed to the filters. Filters
"higher" in the chain are called first, starting at the first
not-yet-connection one. Each filter may add sockets and/or change
flags. When all flags are removed, the socket itself is removed from the
pollset.

Example:

 * transfer wants to send, adds POLLOUT
 * http/2 filter has a flow control block, removes POLLOUT and adds
   POLLIN (it is waiting on a WINDOW_UPDATE from the server)
 * TLS filter is connected and changes nothing
 * h2-proxy filter also has a flow control block on its tunnel stream,

Use of `SEND_HOLD` is no longer necessary in the filters.

All filters are adapted for the changed method. The handling in
`multi.c` has been adjusted, but its state handling the the protocol
handlers' `getsocks` method are untouched.

The most affected filters are http/2, ngtcp2, quiche and h2-proxy. TLS
filters needed to be adjusted for the connecting handshake read/write
handling.

No noticeable difference in performance was detected in local scorecard
runs.

OpenSSL: Include SIG and KEM algorithms in verbose
Currently the verbose output does not include which algorithms are used
for the signature and key exchange when using OpenSSL. Including the
algorithms used will enable better debugging when working on using new
algorithm implementations. Know what algorithms are used has become more
important with the fast growing research into new quantum-safe
algorithms.

This implementation includes a build time check for the OpenSSL version
to use a new function that will be included in OpenSSL 3.2 that was
introduced in openssl/openssl@6866824

- If SSL shutdown is not finished then make an additional call to
  SSL_read to gather additional tracing.

- Fix http2 and h2-proxy filters to forward do_close() calls to the next
  filter.

For example h2 and SSL shutdown before and after this change:

Before:

Curl_conn_close -> cf_hc_close -> Curl_conn_cf_discard_chain ->
ssl_cf_destroy

After:

Curl_conn_close -> cf_hc_close -> cf_h2_close -> cf_setup_close ->
ssl_cf_close

Note that currently the tracing does not show output on the connection
closure handle. Refer to discussion in #11878.

Drop support for "old" / "legacy" / "classic" / "v1" / "mingw32" MinGW:
  https://en.wikipedia.org/wiki/MinGW, https://osdn.net/projects/mingw/
Its homepage used to be http://mingw.org/ [no HTTPS], and broken now.
It supported the x86 CPU only and used a old Windows API header and
implib set, often causing issues. It also misses most modern Windows
features, offering old versions of both binutils and gcc (no llvm/clang
support). It was last updated 2 years ago.

curl now relies on toolchains based on the mingw-w64 project:

OpenSSL: clear error queue after SSL_shutdown
We've seen errors left in the OpenSSL error queue (specifically,
"shutdown while in init") by adding some logging it revealed that the
source was this file.

Since we call SSL_read and SSL_shutdown here, but don't check the return
code for an error, we should clear the OpenSSL error queue in case one
was raised.

This didn't affect curl because we call ERR_clear_error before every
write operation (a0dd9df9ab), but when
libcurl is used in a process with other OpenSSL users, they may detect
an OpenSSL error pushed by libcurl's SSL_shutdown as if it was their
own.

openssl: auto-detect SSL_R_TLSV13_ALERT_CERTIFICATE_REQUIRED
OpenSSL 1.1.1 defines this macro, but no ealier version, or any of the
popular forks (yet). Use the macro itself to detect its presence,
replacing the hard-wired fork-specific conditions.

This way the feature will enable automatically when forks implement it,
while also shorter and possibly requiring less future maintenance.

egd: delete feature detection and related source code
EGD is Entropy Gathering Daemon, a socket-based entropy source supported
by pre-OpenSSL v1.1 versions and now deprecated. curl also deprecated it
a while ago.

Its detection in CMake was broken all along because OpenSSL libs were
not linked at the point of feature check.

Delete detection from both cmake and autotools, along with the related
source snippet, and the `--with-egd-socket=` `./configure` option.

cmake: add support for "unity" builds
Aka "jumbo" or "amalgamation" builds. It means to compile all sources
per target as a single C source. This is experimental.

You can enable it by passing `-DCMAKE_UNITY_BUILD=ON` to cmake.
It requires CMake 3.16 or newer.

It makes builds (much) faster, allows for better optimizations and tends
to promote less ambiguous code.

Also add a new AppVeyor CI job and convert an existing one to use
"unity" mode (one MSVC, one MinGW), and enable it for one macOS CI job.

Fix related issues:
- add missing include guard to `easy_lock.h`.
- rename static variables and functions (and a macro) with names reused
  across sources, or shadowed by local variables.
- add an `#undef` after use.
- add a missing `#undef` before use.
- move internal definitions from `ftp.h` to `ftp.c`.
- `curl_memory.h` fixes to make it work when included repeatedly.
- stop building/linking curlx bits twice for a static-mode curl tool.
  These caused doubly defined symbols in unity builds.
- silence missing extern declarations compiler warning for ` _CRT_glob`.
- fix extern declarations for `tool_freq` and `tool_isVistaOrGreater`.
- fix colliding static symbols in debug mode: `debugtime()` and
  `statename`.
- rename `ssl_backend_data` structure to unique names for each
  TLS-backend, along with the `ssl_connect_data` struct member
  referencing them. This required adding casts for each access.
- add workaround for missing `[P]UNICODE_STRING` types in certain Windows
  builds when compiling `lib/ldap.c`. To support "unity" builds, we had
  to enable `SCHANNEL_USE_BLACKLISTS` for Schannel (a Windows
  `schannel.h` option) _globally_. This caused an indirect inclusion of
  Windows `schannel.h` from `ldap.c` via `winldap.h` to have it enabled
  as well. This requires `[P]UNICODE_STRING` types, which is apperantly
  not defined automatically (as seen with both MSVS and mingw-w64).
  This patch includes `<subauth.h>` to fix it.

connections: introduce http/3 happy eyeballs
New cfilter HTTP-CONNECT for h3/h2/http1.1 eyeballing.
- filter is installed when `--http3` in the tool is used (or
  the equivalent CURLOPT_ done in the library)
- starts a QUIC/HTTP/3 connect right away. Should that not
  succeed after 100ms (subject to change), a parallel attempt
  is started for HTTP/2 and HTTP/1.1 via TCP
- both attempts are subject to IPv6/IPv4 eyeballing, same
  as happens for other connections
- tie timeout to the ip-version HAPPY_EYEBALLS_TIMEOUT
- use a `soft` timeout at half the value. When the soft timeout
  expires, the HTTPS-CONNECT filter checks if the QUIC filter
  has received any data from the server. If not, it will start
  the HTTP/2 attempt.

HTTP/3(ngtcp2) improvements.
- setting call_data in all cfilter calls similar to http/2 and vtls filters
  for use in callback where no stream data is available.
- returning CURLE_PARTIAL_FILE for prematurely terminated transfers
- enabling pytest test_05 for h3
- shifting functionality to "connect" UDP sockets from ngtcp2
  implementation into the udp socket cfilter. Because unconnected
  UDP sockets are weird. For example they error when adding to a
  pollset.

HTTP/3(quiche) improvements.
- fixed upload bug in quiche implementation, now passes 251 and pytest
- error codes on stream RESET
- improved debug logs
- handling of DRAIN during connect
- limiting pending event queue

HTTP/2 cfilter improvements.
- use LOG_CF macros for dynamic logging in debug build
- fix CURLcode on RST streams to be CURLE_PARTIAL_FILE
- enable pytest test_05 for h2
- fix upload pytests and improve parallel transfer performance.

GOAWAY handling for ngtcp2/quiche
- during connect, when the remote server refuses to accept new connections
  and closes immediately (so the local conn goes into DRAIN phase), the
  connection is torn down and a another attempt is made after a short grace
  period.
  This is the behaviour observed with nghttpx when we tell it to  shut
  down gracefully. Tested in pytest test_03_02.

TLS improvements
- ALPN selection for SSL/SSL-PROXY filters in one vtls set of functions, replaces
  copy of logic in all tls backends.
- standardized the infof logging of offered ALPNs
- ALPN negotiated: have common function for all backends that sets alpn proprty
  and connection related things based on the negotiated protocol (or lack thereof).

- new tests/tests-httpd/scorecard.py for testing h3/h2 protocol implementation.
  Invoke:

Improvements on gathering connect statistics and socket access.
- new CF_CTRL_CONN_REPORT_STATS cfilter control for having cfilters
  report connection statistics. This is triggered when the connection
  has completely connected.
- new void Curl_pgrsTimeWas(..) method to report a timer update with
  a timestamp of when it happend. This allows for updating timers
  "later", e.g. a connect statistic after full connectivity has been
  reached.
- in case of HTTP eyeballing, the previous changes will update
  statistics only from the filter chain that "won" the eyeballing.
- new cfilter query CF_QUERY_SOCKET for retrieving the socket used
  by a filter chain.
  Added methods Curl_conn_cf_get_socket() and Curl_conn_get_socket()
  for convenient use of this query.
- Change VTLS backend to query their sub-filters for the socket when
  checks during the handshake are made.

HTTP/3 documentation on how https eyeballing works.

c4cd0e2be9 openssl: Don't ignore CA paths when using Windows CA store (redux)
.. and remove 'experimental' designation from CURLSSLOPT_NATIVE_CA.

This commit restores the behavior of CURLSSLOPT_NATIVE_CA so that it
does not override CURLOPT_CAINFO / CURLOPT_CAPATH, or the hardcoded
default locations. Instead the native Windows CA store can be used at
the same time.

(#5585) but then 83393b1a (#7892) broke it over a year ago, I assume
inadvertently.

lib: connect/h2/h3 refactor
Refactoring of connection setup and happy eyeballing. Move
nghttp2. ngtcp2, quiche and msh3 into connection filters.

 - eyeballing cfilter that uses sub-filters for performing parallel connects
 - socket cfilter for all transport types, including QUIC
 - QUIC implementations in cfilter, can now participate in eyeballing
 - connection setup is more dynamic in order to adapt to what filter did
   really connect.  Relevant to see if a SSL filter needs to be added or
   if SSL has already been provided
 - HTTP/3 test cases similar to HTTP/2
 - multiuse of parallel transfers for HTTP/3, tested for ngtcp2 and quiche

 - Fix for data attach/detach in VTLS filters that could lead to crashes
   during parallel transfers.
 - Eliminating setup() methods in cfilters, no longer needed.
 - Improving Curl_conn_is_alive() to replace Curl_connalive() and
   integrated ssl alive checks into cfilter.
 - Adding CF_CNTRL_CONN_INFO_UPDATE to tell filters to update
   connection into and persist it at the easy handle.

 - Several more cfilter related cleanups and moves:
   - stream_weigth and dependency info is now wrapped in struct
     Curl_data_priority
   - Curl_data_priority members depend is available in HTTP2|HTTP3
   - Curl_data_priority members depend on NGHTTP2 support
   - handling init/reset/cleanup of priority part of url.c
   - data->state.priority same struct, but shallow copy for compares only

 - PROTOPT_STREAM has been removed
   - Curl_conn_is_mulitplex() now available to check on capability

 - Adding query method to connection filters.
   - ngtcp2+quiche: implementing query for max concurrent transfers.

 - Adding is_alive and keep_alive cfilter methods. Adding DATA_SETUP event.
   - setting keepalive timestamp on connect
   - DATA_SETUP is called after the connection has been completely
     setup (but may not connected yet) to allow filters to initialize
     data members they use.

 - there is no socket to be had with msh3, it is unclear how select
   shall work

 - manual test via "curl --http3 https://curl.se" fail with "empty
   reply from server".

 - Various socket/conn related cleanups:
   - Curl_socket is now Curl_socket_open and in cf-socket.c
   - Curl_closesocket is now Curl_socket_close and in cf-socket.c
   - Curl_ssl_use has been replaced with Cur_conn_is_ssl
   - Curl_conn_tcp_accepted_set has been split into
     Curl_conn_tcp_listen_set and Curl_conn_tcp_accepted_set
     with a clearer purpose

openssl: remove attached easy handles from SSL instances
- keeping the "current" easy handle registered at SSL* is no longer
   necessary, since the "calling" data object is already stored in the
   cfilter's context (and used by other SSL backends from there).
 - The "detach" of an easy handle that goes out of scope is then avoided.
 - using SSL_set0_wbio for clear reference counting where available.

openssl: return -1 on error in the BIO callbacks
BIO_read and BIO_write return negative numbers on error, including
retryable ones. A regression from 55807e6. Both branches should be
returning -1.

The APIs are patterned after POSIX read and write which, similarly,
return -1 on errors, not zero, with EAGAIN treated as an error.

 - wolfSSL (v5.0.0 and newer)

 This leaves only the following without HTTPS-proxy support:
 - gskit
 - nss
 - wolfSSL (versions earlier than v5.0.0)

3ae63a121d openssl: silence compiler warning when not using IPv6
In non-IPv6 builds the conn parameter is unused, and compilers which
run with "-Werror=unused-parameter" (or similar) warnings turned on
fails to build. Below is an excerpt from a CI job:

vtls: localization of state data in filters
- almost all backend calls pass the Curl_cfilter intance instead of
   connectdata+sockindex
 - ssl_connect_data is remove from struct connectdata and made internal
   to vtls
 - ssl_connect_data is allocated in the added filter, kept at cf->ctx

 - added function to let a ssl filter access its ssl_primary_config and
   ssl_config_data this selects the propert subfields in conn and data,
   for filters added as plain or proxy
 - adjusted all backends to use the changed api
 - adjusted all backends to access config data via the exposed
   functions, no longer using conn or data directly

cfilter renames for clear purpose:

 - methods `Curl_conn_*(data, conn, sockindex)` work on the complete
   filter chain at `sockindex` and connection `conn`.
 - methods `Curl_cf_*(cf, ...)` work on a specific Curl_cfilter
   instance.
 - methods `Curl_conn_cf()` work on/with filter instances at a
   connection.
 - rebased and resolved some naming conflicts
 - hostname validation (und session lookup) on SECONDARY use the same
   name as on FIRST (again).

new debug macros and removing connectdata from function signatures where not
needed.

adapting schannel for new Curl_read_plain paramter.

lib: connection filters (cfilter) addition to curl:
- general construct/destroy in connectdata
 - default implementations of callback functions
 - connect: cfilters for connect and accept
 - socks: cfilter for socks proxying
 - http_proxy: cfilter for http proxy tunneling
 - vtls: cfilters for primary and proxy ssl
 - change in general handling of data/conn
 - Curl_cfilter_setup() sets up filter chain based on data settings,
   if none are installed by the protocol handler setup
 - Curl_cfilter_connect() boot straps filters into `connected` status,
   used by handlers and multi to reach further stages
 - Curl_cfilter_is_connected() to check if a conn is connected,
   e.g. all filters have done their work
 - Curl_cfilter_get_select_socks() gets the sockets and READ/WRITE
   indicators for multi select to work
 - Curl_cfilter_data_pending() asks filters if the have incoming
   data pending for recv
 - Curl_cfilter_recv()/Curl_cfilter_send are the general callbacks
   installed in conn->recv/conn->send for io handling
 - Curl_cfilter_attach_data()/Curl_cfilter_detach_data() inform filters
   and addition/removal of a `data` from their connection
 - adding vtl functions to prevent use of Curl_ssl globals directly
   in other parts of the code.

curl.h: add CURLOPT_CA_CACHE_TIMEOUT option
Adds a new option to control the maximum time that a cached
certificate store may be retained for.

Currently only the OpenSSL backend implements support for
caching certificate stores.

strcase: use curl_str(n)equal for case insensitive matches
No point in having two entry points for the same functions.

Also merged the *safe* function treatment into these so that they can
also be used when one or both pointers are NULL.

http, vauth: always provide Curl_allow_auth_to_host() functionality
This function is currently located in the lib/http.c module and is
therefore disabled by the CURL_DISABLE_HTTP conditional token.

As it may be called by TLS backends, disabling HTTP results in an
undefined reference error at link time.

Move this function to vauth/vauth.c to always provide it and rename it
as Curl_auth_allowed_to_host() to respect the vauth module naming
convention.

openssl: make certinfo available for QUIC
Curl_ossl_certchain() is now an exported function in lib/vtls/openssl.c that
can also be used from quiche.c and ngtcp2.c to get the cert chain for QUIC
connections as well.

The *certchain function was moved to the top of the file for this reason.
